Case Management

 

If your benefit program provides case management services for large or complex procedures, you need to understand how this process can help you.

 

For major illnesses, how care is delivered and the cost of that care are a complex issues. Case management is meant to assure you that you are getting appropriate care at an appropriate cost.

 

HCPC’s case management nurses work under the direction of HCPC’s medical director. They essentially are your advocate. They are trained to understand the care protocols for major illnesses. In complex cases they often are the primary coordinators of multiple physicians and health care services. Today physicians, hospitals and patients commonly and willingly work with case management nurses to assure a positive outcome.

 

The case management team acts as a resource for the patient and as a financial guardian for the plan.
